To Make An Appointment

 
If you would like to make an appointment for a consultation or to schedule surgery, please contact my secretary, Elinor or Judy at 216-381-2223 or 800-891-5285
 
Prior to making an appointment for a consultation or surgery, I will need information regarding your condition.Please download the Medical Record Release Form, print it out, fill in all the required lines and submit it to the doctors offices and hospitals where you have received care.It is essential that I review your old records prior to your office visit.I also would like you to download The Questionnaire regarding your medical condition.After downloading the questionnaire, please fill it in legibly and fax it to my office at 216-381-5975.Once I have reviewed your questionnaire and your old medical records, my office will contact you to schedule a ten minute (no-charge) phone consultation, after which I should be able to assess the kind of evaluation that you will need.It is essential that you be available to speak to me on the phone at our scheduled phone consultation.
 
If you are planning to have the surgery performed by me and you are coming from outside Ohio or the United States, I will usually schedule your office visit one day prior to your scheduled surgery.You will need to do the bowel prep, prior to your office visit and continue it after your office visit until the day of the surgery.You may download instructions for The Bowel Prep from this Web site. If you are planning to have the surgery, you should travel to Cleveland with a companion, so he or she can provide you with a transportation to your hotel after the surgery.
 

Payment Options

 
If you reside in the metropolitan Cleveland area or Northeast Ohio, we will usually accept your insurance as payment, unless we are not on your provider panel. If you reside outside Northeastern Ohio, we will ask you to make financial arrangements prior to your visit. We will file your insurance claim but you will be responsible for the entire balance.Toobtain additional information including our fee schedule, please contact my office and speak to my secretary Elinor or Judy at 216-381-2223 or 800-891-5285

The Cleveland Hopkins International Airport is a hub for Continental Airlines. At the same time, all the other major domestic airlines are represented at the Cleveland airport.Taxi fare from the airport to hotels is usually from $25 to $30.I suggest that you rent a car, as taxi service in Cleveland is notoriously unreliable.
